What is the Clover Leaf Intensive Supports Program (CL-ISP)?

The George Spady Society Cloverleaf Manor Intensive Supports Program (CL-ISP) is an innovative partnership with the Leduc Regional Housing Foundation and Alberta Health Services.

CL-ISP is located in Warburg, 90 kilometers south east of Edmonton and serves individuals in an inclusive communal living setting with a focus on wellness recovery. Cloverleaf Manor provides residents with daily meals, satellite tv access, weekly light housekeeping, and linen exchange. 

Services Provided

Up to 13 residents can be housed in Wing 1 where CL-ISP is based. Individuals in the program require a higher level of non-medical personal care supports and enjoy living in and contributing to a warm inclusive community environment.

  • Support services include 24/7 staffing, coordination of appointments and related transportation, and daily medication administration.

  • Case Coordinators and Alberta Health Services provide integrated case management services to enhance individuals’ health.

  • Mental health and addictions support with a harm reduction focus on site.

  • Activities such as smudge ceremonies, mindfulness sessions, creative arts, and walking groups encourage community engagement.

  • Participants have opportunities to travel to nearby communities to enjoy recreation and shopping.

Referrals

Referrals to CL-ISP begin with service providers forwarding an assessment to an Alberta Health Services Intake Coordinator.

All applications for residence in permanent supportive housing are screened to ensure applicants meet eligibility requirements. Following the initial application screen, a multidisciplinary team will review the applications to determine:

  1. If the applicant requires further assessments to determine level of support

  2. If the applicant’s needs match the services offered

  3. Appropriate community resources and supports for the applicant if permanent supportive housing services are not appropriate
